This form must be completed and medical records sent to VCA South Shore in order to schedule an appointment. A referral coordinator from VCA will contact your client to schedule an appointment. Patients need to be medically stable and compliant/cooperative without sedation when referred for ultrasound. If the patient is not compliant, an exam and sedation may be necessary to obtain the ultrasound.

When you submit this referral form, our referral office will call the owner to schedule the appointment.

Give the following instructions to the client

  • Fast the pet for 12 hours prior to the appointment, unless diabetic or otherwise contraindicated; Access to drinking water is okay.
  • The hair coat will be shaved, as needed for the exam.
  • Owners can not be present during the ultrasound. If the patient becomes uncooperative, an exam and sedation will be required in order to obtain the ultrasound.
  • The radiology report will be sent directly to referring veterinarian.
  • Radiologist will not discuss findings with owner.

Instructions for referring veterinarian:

  • Once you complete this referral form, the referral office will contact the client to start the scheduling process.
  • If you are unable to upload diagnostic images or test results, please send radiographs and laboratory test results with the owner or email them to SouthShoreReferral@vca.com
  • The radiology report will be sent directly to referring veterinarian.
  • Radiologist will not discuss findings with owner.
  • Patients need to be medically stable and compliant/cooperative without sedation when referred for ultrasound. If sedation is required, an exam and sedation will be charged at the time of the visit.
  • Referring veterinarian must be available to discuss findings with the radiologist on the day of the exam. Please provide an alternate phone number if your practice is closed or you will be out of the office.
  • In an urgent situation, if your hospital is not available, additional inpatient services will be offered to your client.
